Perimenopause is often a busy, joyful, and sometimes confusing time for many women.  As you transition from regular ovulatory cycles through perimenopause into  menopause, significant hormonal changes can profoundly impact your health and daily  life. During regular cycles, estrogen and progesterone follow a predictable pattern,  supporting ovulation and regulating menstruation. Menopause, defined as 12

consecutive months without a period, typically occurs between the ages of 45 and 55,  marking the end of ovulation and a sharp decline in estrogen and progesterone.

Perimenopause, the transition phase, brings unpredictable hormonal fluctuations,  especially in estrogen, as ovulation becomes irregular. This phase, which can begin as  early as the mid-30s, often leads to irregular periods, hot flashes, mood swings, vaginal  dryness, and sleep disturbances. Testing hormone levels during this time if not often  useful, as levels can vary significantly from day to day.

Hormone Replacement Therapy (HRT): When to Start?

One of the number when questions I get asked as a Naturopathic Doctor is “When  should I start HRT?”. While HRT is a safe and effective intervention for many women with  hormonal concerns, it is often not recommended to start until menopause. During  perimenopause, HRT is generally reserved for more extreme cases.

So, where does that leave perimenopausal women, especially those who want to be  proactive in their health?

Herbal medicine and supplements provide a natural approach to managing  perimenopausal symptoms, especially when HRT is not yet indicated Black cohosh, red  clover, and maca are well-studied herbs known for their support in balancing estrogen.  Adaptogenic herbs like ashwagandha and rhodiola can help manage stress by balancing  cortisol levels, which often fluctuate due to changing estrogen and progesterone. These

adaptogens also support adrenal health, maintain energy levels, and stabilize mood  during this transition phase.
