In this article we'll compare two increasingly popular approaches: barefoot training and orthotics, and try to help you decide what’s best for you. Additionally, we'll discuss a potential timeline for those who want to harness the advantages of both approaches.

Barefoot Training:

1. Workload and Time Expectation:

Barefoot training involves retraining your feet and lower limbs in a way that allows them to give you more mechanical feedback and increase your balance, stability and ability to generate power. This approach demands a significant commitment of time and effort, including:

- Gradual transition: Shifting from traditional footwear to barefoot shoes or going completely barefoot should be a gradual process taking place over months. This ensures that you safely adapt to this new way of walking, running and lifting mitigating injury risk.

- Strengthening exercises: I'm going to prescribe a variety of exercises designed to strengthen the muscles in your feet, calves, and lower legs. These exercises require consistent effort and practice.

- Patience: Adjusting to barefoot training can take several weeks or even months, depending on your previous footwear habits and the condition of your feet. We can do regular check-ins to monitor your progress, and have several activity based benchmarks to predict where you can safely push the envelope of progression. 

2. Ideal Patient Population:

Barefoot training with your chiropractor is suitable for individuals who:

- Are committed to dedicating the necessary time and effort for successful retraining.

- Don't have underlying foot conditions that might hinder the retraining process.

- Have long standing knee, hip or back problems that would be aided by enhanced stability.

- Prefer a natural, minimalist approach to footwear

- Love the idea of building bulletproof, supple and stable lower limb movements

Orthotics

1. Workload and Time Expectation:

I prescribe orthotics to provide a more immediate solution to lower limb issues with a relatively lower workload and time expectation:

- Consultation: I begin by assessing your specific lower limb condition and needs. Your chiropractor can then prescribe custom orthotics tailored to your unique requirements.

- Fitting and adjustment: Once your custom orthotics are ready, I will assist in fitting them into your existing footwear. Minor adjustments may be necessary, but this process is typically quick.

- Immediate relief: Orthotics are designed to provide instant support and alleviate discomfort, requiring minimal time for adaptation (weeks vs. months).

2. Ideal Patient Population:

Orthotics are particularly beneficial for individuals who:

- Have active diagnoses in the lower limb, such as plantar fasciitis, flat feet, or bunions.

- Require immediate relief from foot pain without the need for extensive retraining.

- Cannot tolerate a retraining program due to time constraints or other limitations.

- Stand or walk for extended periods (e.g., more than 8 hours a day) and need constant support.

Combining the Best of Both Worlds!:

1. Initial Consultation with Your Chiropractor: We do a chat and assessment to identify your specific conditions and needs.

2. Orthotics for Immediate Relief: If immediate relief is required, I’ll immediately fit you for a custom orthotic. These can be used as you gradually transition to barefoot training.

3. Barefoot Training with Chiropractic Care: Begin the process of barefoot training gradually, with expert support and guidance. You’ll focus on strengthening and mobilizing your feet and lower limbs while receiving comfortable hands on care to speed your recovery.

4. Gradual Reduction in Orthotic Use: As you gain strength and stability we’ll taper off the use of orthotics and care to maintain your comfort.
